N-O-T: Not On Tobacco

Online N-O-T Application


Read these guidelines FIRST. Then fill out the online application using the button below.

To receive funding and support for N-O-T, applicants must agree to comply with the following guidelines:

Facilitators

  • Facilitators must complete or have previously completed the American Lung Association of Colorado’s (ALAC) N-O-T facilitator training prior to the start of N-O-T groups.
  • All facilitators must be non-tobacco users for at least 6-months.
  • At least 1 N-O-T facilitator from each school/organization must attend the Statewide Facilitator Summit in April 2009.
    • You must send one representative from your school/organization in order for all facilitators to receive their stipend.
Program Implementation
  • Facilitators will use the N-O-T program as it was designed without significant change or alteration (may not be shorter than 5 weeks in length).
  • The N-O-T program is conducted on school grounds during school hours, unless otherwise permitted by ALAC.
  • If the N-O-T program is offered for credit, N-O-T participants will not receive a grade for participation, but rather “credit/no credit.”
  • Program participants must remain confidential.
  • The N-O-T program is voluntary, not punitive (i.e., cannot be used in lieu of suspension, ticket, or fine for being caught smoking).

Reporting

  • Students' pre- and post-tests on Sessions 1 and 10, must be returned to ALAC by June 1, 2009.
  • Participant attendance form and a budget reconciliation form showing how funds were spent must also be returned to ALAC by June 1, 2009. Integrity with N-O-T budget and incentives is a must.
  • Facilitators must submit 2 online reports for the year:
    •  Implementation report
    •  Final report due May 15, 2009

What You Will Receive
Upon approval of the 2008-09 N-O-T application, schools/organizations will receive:

  • N-O-T facilitator training and curriculum (one-day training)
    Student workbooks and journals for all participants
  • Free student incentives such as water bottles, golf-size pencils, DVDs, posters, and other tobacco educational materials
  • Funding via check to the school to cover additional student incentives, substitute teacher costs and travel costs. Funding must be used by May 15, 2009.
  • Facilitator stipends of $100 for each facilitator each time the N-O-T program is implemented provided one representative from your school/organization did attend the N-O-T Facilitator Summit.
